WINTER Athletic Registration - FinalForms

 

We are very excited to announce that the Dover School District Athletic Department will continue using FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your student(s). The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. As a result, if you have registered your child(ren) for a sport during previous seasons, your information will still be in FinalForms.

 

You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

 

 We will accept service fee payments after teams are picked and final cuts happen. An email will be sent home when the payment option is open.

 

Physicals should be uploaded electronically on FinalForms during the registration process.

 

We require that all parents/guardians of student-athletes use FinalForms to register for our athletic programs.

 

On-line registration will begin on:

 

• November 3, 2025 for all Winter of 2025 Dover High School and Middle School Athletic Programs

 

Please register at:  https://dover-nh.finalforms.com/ and follow the prompts to create your account, create your students, and sign your forms.  If you require any support during the process, scroll to the bottom-right of the FinalForms page and click “Get Help”.

 

 

DOVER HIGH SCHOOL and MIDDLE SCHOOL – 2025 Winter Service Fee Information:

 

 

The Service Fee is $150 for DHS Athletics participation and $80 for DMS Athletic participation.

 

* Reduced Lunch Student fee is $75 for DHS students and $40 for DMS students.

 

** Free Lunch Students will not be charged a fee but MUST click on “Fee Waiver” option and type the reason in the box provided (free lunch recipient).

 

There is a “fee waiver” option if this fee creates a financial hardship. Please communicate with the Athletic Director via email [email protected] to discuss this financial hardship. We STRONGLY prefer that payments are made by Credit Card through your FinalForms account (be aware there is a 4% transaction fee). 

 

If you choose to pay by check. Checks need to be made out to “Dover School District” and mailed/brought to Dover High School – 25 Alumni Drive – Dover, NH. 03820 - ATTN: Athletics Department. **Please list which sport your child is playing in the memo/subject line of check.

 

WE WILL NOT ACCEPT CASH!

 

**Once you have paid for two sports during the 2025-26 school year, per athlete, the third sport is Free.
